Declaring a Disability. The Academic Services Center coordinates all disability accommodations at High Point University. In order to receive accommodations, students are required to declare their disability to the ASC Director. It is the students' responsibility to declare a disability. While faculty may encourage students to do so, students are not required to self-identify. Prior to review of documentation, students must complete and sign the ADA Intake Form. The form asks for current contact information and requested accommodations. It also authorizes the ASC Director to release disability-related information to campus officials as necessary.

Documentation. Diagnostic documentation must meet the following criteria:

  • Evaluation within the past three (3) years
  • DSM-IV diagnostic code, if appropriate
  • Statement of impact on the person's life as a student in higher education
  • Recommended accommodations
  • Evaluator appropriate for diagnosis
  • Evaluator's qualifications and signature on letterhead with written evaluation
